Job Description

Avante Group, Inc. is seeking a Nurse Marketing Liaison to foster strong, effective and efficient care relationship between facility residents, family members, facility staff and other healthcare providers in the local community. The NML serves as an advocate for residents beginning during the pre-admissions process and continues throughout the resident's stay at an Avante facility and after the resident is discharged. In Avante's long-term care and rehabilitation environment, the NML establishes a resident's eligibility for care, communicates with families and interacts with a wide range of facility staff members, from Directors of Sales and Marketing, to Internal Admissions Coordinators, to healthcare providers, and Avante's regional marketing team members. The NML is often the first to assess potential residents, reviews medical records, and determines whether a potential resident should be admitted for care to an Avante facility. The NML explains the types of care and services Avante facilities offer to residents and their family members. The NML also works with other facility staff and outside providers to schedule physician visits and other medical-related sessions outside the facility, if needed. The NML verifies a potential resident's health insurance and its coverage (i.e., private health insurance, Medicare, Medicaid). The NML will coordinate with other facility staff members to ensure smooth resident discharge and transition to home or other medical facility. Duties of the NML also include training of other facility staff members in the NML's area(s) of expertise.

This opening is for Mon - Fri, shift 8:30 AM - 5 PM. 

Job Requirements

Qualifications:
• Must be a Registered Nurse (RN) in good standing and meet all applicable federal and state licensure requirements;
• Must be licensed in Florida;
• Must be flexible and willing/able to travel, as needed;
• Must possess the ability to make independent decisions when circumstances warrant such action;
• Must promote goals of care across the continuum with a working knowledge of the resident, and must facilitate timely access to appropriate resident care;
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public;
• Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult or challenging residents and family members;
• Must be able to relate information concerning a resident's condition;
• Must show ability to read and write in English in a legible manner, and speak English in an understandable manner;
• Must have experience in establishing solid communication structures, proactive relationships, and defined goals coordinated with the resident, resident's family/responsible payor, Interdisciplinary team members, primary care provider(s) and local community resources;
• Must have experience with medication reconciliation across transitional facilities;
• Must have experience in participating and/or assisting with the oversight and
adherence to a short-term care plan, comprehensive care plan and discharge plan in conjunction with the resident, resident's family/responsible payor, facility staff, Interdisciplinary team members, primary care provider(s) and local community resources;
• Must have previous experience in a hospital, long term care facility or other
healthcare-related facility with similar role and responsibilities;
• Must be knowledgeable of general, rehabilitative and restorative nursing and
medical practices, procedures and laws, regulations and guidelines governing the long-term care industry;
• High degree of honesty and integrity required;
• Positive attitude toward the elderly;
• Must be at least 18 years of age;
• Must be able to work in a constant state of alertness and in a safe manner at all times.

 

Education and Experience:
• Registered Nurse (RN) Degree required;
• Bachelor of Science Degree in Biology, Life Science, Anatomy or related field
strongly preferred, but not required;
• Minimum 3-5 years' experience in business development required;
• Minimum 3-5 years' experience as an RN required in clinical or community
resource settings required;
• Evidence of essential leadership, communication, education and counseling skilled required;
• Proficiency in communication technologies (i.e., email, cell phone, etc..), including Microsoft Office applications, and electronic medical records systems (i.e., health care related Information Technology (IT) systems) required;
• Experience with managed care, insurance processes and public/commercial payer sources preferred;
• Current local knowledge about and connections to community health care and
social welfare resources preferred;
• Ability to speak a relevant second language is preferred.
